In Rajasthan, 1 out of 2 married women uses modern contraceptives methods for family planning

IIHMR University implements the project on Population and Reproductive Health in the state 


·         4874 households and 297 health facilities were surveyed from June-September, 2016
·         Usage of contraceptives: Use of  Birth control pill is up from 5% in 2005-06 to 7% in 2016, Male condoms are used by 10% of married women which is up from 6% in 2005-06
·         The proportion of couple who do not want another child is higher than those who want to delay childbirth for two years or more
·         The total unmet need for family planning in married women in Rajasthan between 15-49 years is 15% (9.5% is for limiting [those who do not want another child] and 6% is for spacing[those who wish to give a gap and produce another child])
·         Poor women have more unintended pregnancies than wealthy women
·         More than ¾ of public facilities i.e 77% offer at least 3 modern contraceptives and 38% offer 5 or more contraceptive methods
·         About 96%  of Community Health Centers and 87% of Primary Health Centers offer 3 or more contraceptive methods, while 92% of public hospitals offer 5 or more methods
·         Median age at first marriage (25-49 years)  is 18.5 years.
·         Median age when women start to use contraception is 22.2 years

One of the globally acclaimed universities and renowned research institutions, IIHMR University, Jaipur, has been implementing project PMA2020 in Rajasthan. Performance Monitoring and Accountability or PMA2020 involves India as one of the program countries out of the other 10 countries which include, Ghana, Ethiopia, Kenya, Burkina Faso, Indonesia, Nigeria, Uganda, and Democratic Republic of Congo. The project is implemented by IIHMR University, Jaipur in collaboration with Bill & Melinda Gates Institute for Population and Reproductive Health  at the Johns Hopkins Bloomberg School of Public Health and International institute for Population Sciences and Ministry of Health & Family Welfare. . Project is funded by the Bill and Melinda Gates Foundation. .
According to Dr. Anoop Khanna, Principal Investigator, Rajasthan for PMA 2020 and Professor at IIHMR University “Performance Monitoring and Accountability 2020 (PMA2020) uses innovative mobile technology to routinely gather rapid-turnaround, cost-effective population data on family planning and water, sanitation and hygiene.” He added, “PMA 2020 is a project that focuses on important indicators that monitor family planning. The project has recently concluded round one and will provide information every 6 month till 2017 and once a year from 2018. Round one included 4874 households and 297 health facilities which were surveyed from June-September, 2016 on reproductive health in the state. It has been observed that in the state of Rajasthan the overall family planning has been given more importance in the recent years.”
According to the project findings, more than ½ of married women are using modern contraceptive methods, where the use of birth control pill is up from 5% in 2005-06 to 7% in 2016 and the use of injectable has increased.  Male condoms are used by 10% of married women which is up from 6% in 2005-06. Although Female sterilization is the most common form of contraception, it has decreased over the last decade. 
According to the project findings in round one Dr. Anoop Khanna further added, “The current level of unmet need for family planning in Rajasthan is 15% which is slightly below the level 10 years ago which shows that the overall demand for family planning is increasing. The unmet need for limiting (i.e those who do not want another child) has increased while the unmet need for spacing (i.e those who wish to have another child) has slightly declined. This presents an opportunity to expand access to long-acting methods, especially reversible methods, to enable women to meet their reproductive goals. Thus, the total unmet need for family planning in married women in Rajasthan between 15-49 years is 15% where 10% is for limiting and 6% is for spacing.”
Inequities are not just a part of the income being distributed but these inequities are also found at the family planning level where the report findings specify that poor women have more unintended pregnancies than wealthy women. Among women in the poorest wealth quintile, 22% reported that their last pregnancy was unintended, compared to 12% in the wealthiest quintile. Increased access to contraception, and especially highly effective long-acting methods like IUDs and implants, may help prevent unintended pregnancies, particularly among poorer women.
Even though multiple healthcare facilities offer contraceptive facilities, for many women other more effective reversible methods remain out of reach. More than three-fourths of public facilities offer at least 3 modern contraceptive methods. Among public facilities, 77% offer at least 3 modern contraceptive methods and 38% offer 5 or more modern contraceptive methods. About 96% of Community Health Centers (CHCs) and 87% of Primary Health Centers (PHCs) offer 3 or more methods, while approximately 92% of public hospitals offer 5 or more methods.
The findings of the project on Adolescent Reproductive health specifies that Young women start to use family planning several years after they become sexually active and, often, after they have already given birth. The median age when women first start using contraception is 22.2 years. However, the median age at first sex is 18.3 years and the median age at first birth is 21.0 years. The pattern is even more pronounced when looking specifically at women living in urban areas who have their first birth at age 21.0, but do not start using family planning until almost three years later, at age 23.1. By that time, women in urban areas have, on average, 2.1 children. This suggests that unmet need for young, urban women may be especially high, and presents an opportunity to expand family planning offerings and increase contraceptive coverage for this group.  

In Rajasthan, data collection is led by the Indian Institute of Health Management Research (IIHMR) University in Jaipur. A sample of 147 enumeration areas (EAs) was drawn by the International Institute for Population Sciences from a master sampling frame. In each EA households and private health facilities were listed and mapped, with 35 households randomly selected in each EA. Households were surveyed and occupants enumerated. All eligible females ages 15 to 49 were contacted and consented for interviews. The final completed sample included 4,874 households (97.2% response rate), 5,262 females (96.0% response rate) and 297 health facilities (96.1% response rate). The first round of data collection was conducted between June and September 2016.

The WCO-India Programme Of Work Supports The GOI In
  • To develop strategies for prevention and control of vaccine preventable diseases;
  • Formulate evidence based public health policies and intervention;
  • Strengthen disease surveillance and risk factors and monitoring implementation of integrated communicable disease prevention;
  • Promote equity of health care;
  • Capacity building through training manpower;
  • Building partnerships and develop stronger networks through liaison between concerned departments/Ministries/institutions responsible for execution of GoI/WHO collaborative programme.

    All the Member States of WHO in the South East Asia Region share the common value of the highest attainable standard of health as a fundamental human right. All of WHO actions are based on this and rooted in the underlying values of equity, solidarity and participation.

    The seven point global health agenda contained in WHO's Eleventh General Programme of Work (2006-2015) includes: (i) investing in health to reduce poverty; (ii) building individual and global health security; (iii) promoting universal coverage, gender equality and health related human rights; (iv) tackling determinants of health; (v) strengthening health systems and equitable access; (vi) harnessing knowledge, science and technology; and (vii) strengthening governance, leadership and accountability.

    The South -East Asia Region of WHO is made up of 11 countries, with over 1.8 billion people, with India's population of 1.2 billion. The diversity of people and health situations requires, on the demand side, a solid understanding of the emerging socioeconomic, epidemiologic and demographic patterns and trends at both aggregate as well as national and sub-national levels, with due sensitivity to disadvantaged populations and vulnerable segments of the society. On the supply side, it is equally crucial to have a solid understanding of the commonalities and divergences of the health systems, the circumstances and political economy under which they have evolved and operate.

    Mission of the WHO Country Office for India

    Within the framework of WHO's Country Cooperation Strategy the mission of the WHO Country Office is to support India in: (i) developing and sustaining its own health policies, health systems and public health programmes; (ii) working to prevent and overcome threats to health; (iii) anticipating future challenges; and (iv) advocating public health.

    Description Of Duties
    Under the overall guidance of the WHO Representative to India, the direct supervision of Deputy Project Manager-Technical, with a view to help achieve the expected results set out in the relevant WCO-India biennial work plans in accordance with WHO's Country Cooperation Strategy with India (2012-2017), as part of a broader WCO-India team working on Universal Health Coverage goals the incumbent will have the following responsibilities:

  • Provide technical oversight and strategic guidance for establishment of laboratory supported surveillance for vaccine preventable diseases in India;
  • Provide strategic oversight for using the experience and best practices of surveillance for poliovirus detection to establish surveillance for other vaccine preventable diseases so that the benefits of polio eradication can be used for broader health system strengthening at primary health care settings;
  • Ensure strategic guidance to maintain poliovirus surveillance during the implementation of the polio endgame strategy as a part of the Global Strategic Plan 2013-2018;
  • Support the collation, management, analysis and use of surveillance data on the epidemiology of vaccine preventable diseases to drive decision making for policy development;
  • Contribute to operations research, evaluation and special studies related to control of vaccine preventable diseases and effective program strengthening activities;
  • Coordinate and supervise WHO activities related to capacity building for establishing and maintaining surveillance for vaccine preventable diseases;
  • Coordinate and work closely with WCO-India Health Systems and Communicable Disease teams as well as with all the partner agencies including UNICEF, Rotary and other relevant agencies on surveillance for vaccine preventable diseases as part of a broader Universal Health Coverage goal;
  • Develop policy documents, guidelines, annual work plans and budgets for surveillance of vaccine preventable diseases in the country. Provide technical assistance and support in the development and adaptation to the country context of the relevant WHO normative materials and guidelines;
  • Write and review reports on the quality of surveillance for vaccine preventable diseases for internal WHO review as well as for sharing with government and partners;
  • Develop proposals for donor funding for surveillance of vaccine preventable diseases;
  • Support information brokering/exchange function of WCO India through contributions to the health information repository at the WCO India by collation, analysis and sharing of relevant health information and statistics related to surveillance of vaccine preventable diseases;
  • Represent WHO-India as a designate member in technical programme and performance reviews to ensure sensitive surveillance for vaccine preventable diseases;
  • Contribute to the external relationships, representation, communication and advocacy work of the WCO-India aimed at improving health system transparency and accountability;
  • Ensure clear and accountable work plans linked to adequate financial and human resources are developed for vaccine preventable diseases and ensure implementation and impact; oversee project budgetary planning and resource mobilization activities;
  • Prepare draft reports, strategic documents and programme updates for policy makers and partners, and ensure the quality of programme bulletins, surveillance reports and other scientific publications related to vaccine preventable disease surveillance;
  • Support the monitoring of utilization of donor funding ensuring that the funding is utilized efficiently and appropriately as per WHO rules and regulations and that reports to donors are produced on time and according to the requirements of the respective donor agreements;
  • Submit a report of work done to the WHO representative at the end of each biennium.
  • Education
    REQUIRED QUALIFICATIONS

    Essential: University Degree in Medicine.

    Desirable: Postgraduate degree or training in Public Health/Epidemiology/Public Health Administration/Organization of health
    services or control of Communicable Diseases.

    Good knowledge of the administrative functioning of government institutions and related departments/agencies, and of the other
    partners in India.

    Sound technical and policy advisory skills, based on evidence. Public health leadership skills with demonstrated ability to work effectively with government and colleagues in a team setting at national and international levels, to share information and data and make oral and written presentations on technical issues. Very good ability to build and maintain relations and network with national authorities and other stakeholders across relevant sectors. Understanding of the potential motivating factors within national context and ability to adjust to new approaches in an increasingly complex environment.
    Modern management skills (planning and evaluation, etc).Capacity to prepare terms of reference and to prepare and monitor and manage the implementation of contractual agreements. Capacity to convey information and options in a structured and credible way; ability to speak and write clearly. Proficiency in computer applications and ability to draft reports.

    Knowledge of WHO /UN agencies programmes and practices will be an advantage.
